About the Community Navigator - Case Manager 

As part of a small team, on a day to day basis you will:

  • Provide intake and assessment services with participants
  • Assist participants to develop evidence based support plans with actions, goals and time frames
  • Provide opportunities for participants to develop life skills
  • Provide advocacy and peer support to participants
  • Refer participants to support services
  • Establish and sustain community networks/partnerships

To enjoy this role, you will have:

  • A qualification in mental health
  • Experience in working with people with a mental illness
  • Demonstrated experience in managing participants with difficult behaviours
  • Experience in facilitating group forums and organising events and training
  • The ability to practice strength-based recovery approaches
